Mentor Program Application Process
INTERVIEW
Heart Gallery Mentor Program Director, Lisa Long will contact you and set up an interview.
MATCHING PROCESS
Foster youth will be referred by caseworkers, They will need a completed referral form filled out to enter the program.
Foster parents and foster youth must sign and return individual forms.
Review foster youth's forms and select a good match for the prospective mentor.
First meeting with foster youth
The mentor will meet with the foster parent(s) and youth with(out) the caseworker, depending on what is right for the situation. The mentor and foster parent(s) will discuss expectations and rules, develop a plan for regular visits and communication. The mentor director will aid in the initial gey to know you phase before leaving and letting the connection begin.
Mentors and foster youth visits
There needs to be at least 10 hours of monthly visists and can be more. We strongly suggest more during the intitial relationship building period.
Follow-up and on-going training
Monthly activity report is to be filled out by the mentor and returned to the Heart Gallery Mentor Director.
The Heart Gallery Mentor Program Director will send a occasional summaries of the mentor and youth's activities, progress and difficluties to the caseworker.
