Forms Needed to Begin Volunteering

CASEWORKERS - Do you need a lifebook created for a foster child on your case load?

Caseworker Lifebook Request

VOLUNTEERS - These books will impact the lives of these children and protect memories that otherwise might be lost. These will be a cherished memory forever.

There are three stages of volunteering for this project.

1. Making stock pages...Birthday, holiday, sports, etc. No photos, script or story included
This stage requires steps 1 & 2 as well as the volunteer time sheet

2. Making pages with photos and script
This stage requires steps 1, 2, 3, and 4 as well as the volunteer time sheet

3. Making pages with photos, script and child's story
This stage requires steps 1, 2, 3, and 4 as well as the volunteer time sheet

1. Volunteer Application - Required

2. Background Check Form for Volunteers - Required

3. Confidentiality Form - Required

4. Pre-Registration is required through the DHS website - Required

Volunteer Time Sheet - DHS is required by the State of Oregon to record
volunteer hours, even for Life Story Bookers.
