Heart Gallery Adoption Agency Fee Schedule

Home Study Application Fee                                                      
$300

Home Study plus two committee representations           
$1500 plus travel time and mileage

Additional committee representations                                 
$200 each

Post-placement Supervision                                                      
$0 (paid by the state)

Travel Time                                                                                        
$10/hour
Mileage                                                                                               
.25/mile

Fee for service, as listed above, becomes due and payable at the time the service is requested.  The application fee is due at the initial meeting.  The home study fee is due at the second meeting.  Fees are subject to change after 90 days.
